Edgewood Manor Rehabilitation and Healthcare Center Social Service Director Lisa Moore (center) received an Ohio Health Care Association Skilled Nursing Excellence Award for her dedication to the industry. Joining her at the awards ceremony were Edgewood Manor Administrator Lori Opfer, and Lisa’s father Robert Hughes.

Ohio’s largest long-term and post-acute care organization recently honored Lisa Moore, LSW, Social Service Director, of Port Clinton’s Edgewood Manor Rehabilitation and Healthcare Center as a 2023 OHCA Skilled Nursing Excellence Award Winner during special ceremonies in Columbus.

The Ohio Health Care Association (OHCA) presented the award to Moore during the Association’s 2023 Fall Conference as part of the 2023 OHCA Excellence Awards, which recognize that quality professionals are the backbone of the long-term care community. There were over 100 applicants nominated and the association awarded six individual awards as well as two team awards this year.

Edgewood Manor Administrator Lori Opfer presented Moore, recognizing her for giving over 12 years of service and dedication to the facility. Opfer shared Moore’s dedication to each resident by noting, “To say Lisa goes above and beyond is an understatement. She takes her job very seriously and treats each resident and family member like they were her own mother, father, son or daughter.

“Through her 12 years at Edgewood, she has diligently ensured every discharged resident has all the equipment and supplies to return to the community safely,” Opfer added. “She has done numerous referrals to the Ohio HOME Choice housing transition program and with several of these referrals, has personally gone with the resident to their new home or apartment to ensure they have everything set-up for a safe discharge.”

Moore received her Master’s degree from Ohio State University in Geriatric Counseling. She has been in long term care for 33 years and at Edgewood Manor 13 years.
